Receiving provider declaration
This form accompanies a submission to the Education Standards Board to receive transfer of an approved provider.
It should be completed by: an individual applicant, company director or secretary, public officer and one other member of a managment committee (Incorporated association), director of a cooperative, managing partner, appointee according to rules of a corporation or a government school council.
Once complete, attach the form to the Receiving provider application.
